Smiley’s Family Medicine Clinic
Eligible patients can receive coordinated behavioral and medical care, known as your “Behavioral Health Home.” Your coordinated team includes a behavioral health provider, social worker and community health worker. M Health’s Primary Care Behavioral Health Home team works closely together to help you work through day-to-day challenges and move toward healthy living.

Transgender Health Directory
The Minnesota Transgender Health Coalition and Rainbow Health Initiative have collaborated to produce a new updated LGBTQ-friendly health provider directory! The purpose of the directory is to list all kinds of providers, from doctors and nurses to chiropractors and dentists, all of whom agree that they are personally and professionally supportive of the LGBTQ community.
